A policyholder authorises an auto-debit but the bank rejects it for insufficient balance. The insurer app still shows the policy in grace. The family assumes the debit will retry automatically. The policy later lapses. The correct action is to obtain an official receipt or status confirmation, not rely on a pending bank instruction.

How to apply the framework

Maintain a premium calendar and reconcile every payment. If the amount is debited but no receipt is issued, contact both bank and insurer with the transaction reference. Do not pay again through a caller’s personal link. For life policies, compare revival with paid-up or surrender implications; for health policies, protect continuity and waiting credits.

A revival is not complete merely because money is accepted. The insurer may require underwriting and must communicate acceptance under its process. Disclose current health truthfully. After revival, download the endorsement/status letter and check riders. If a claim event occurred during the gap, obtain professional advice before making representations.
