When and how this form is used

Rule 4’s mine proviso requires at least 30 days’ prior notice for the listed mine events. It differs from the post-event 30-day construction notice.

Complete the correct event branch, responsible-person contacts, regulatory plans and safety-management plan. A notice is not permission to disregard mining lease, environmental, safety or closure requirements.
