When and how this form is used

Rule 4(1) requires the covered construction/contract-work employer to submit within 30 days of commencement or cessation. The sample’s future date is a training assumption, not a filed notice.

For cessation, rule 4(2) requires the dues and hazardous-substance certificates. For a mine, the proviso uses Form VII with at least 30 days’ prior notice, so the timing is different.
