When and how this form is used
The2022 amendment substitutes45 days in Rule17A, rather than the15 days printed in the2020 source. The example uses1 October2026 as the local/bank event date and20 October as intimation:19 elapsed days. Rule17A’s final-approval proviso is separately disclosed; the model does not certify that MHA has approved or made the FCRA change effective.
The22 June2026 Rule2(ca) key-functionary definition includes directors, partners, trustees, Karta, controlling-body members and persons controlling/managing affairs. Checking only the three officer titles on a letterhead can miss an in-scope person. This example assumes the three listed persons are the complete controlling body.
The form requests details of all key members after change, rather than only the person added. The completed model includes the retained President and Treasurer as well as the proposed Secretary.
The2025 note requires the prior governing-body resolution and Proforma AA for each person added/modified. Its added citizenship/OCI, conviction and pending-prosecution paragraphs are separate checks. The2026 eligibility explanation concerning foreign nationals is not used to reject or approve an Indian-citizen model without evidence.
