When and how this form is used
The2022 amendment substitutes45 days in Rule17A, rather than the15 days printed in the2020 source. The example uses1 October2026 as the local/bank event date and20 October as intimation:19 elapsed days. Rule17A’s final-approval proviso is separately disclosed; the model does not certify that MHA has approved or made the FCRA change effective.
The2020 substituted FC-6A says name/address; the older2019 within-the-State label must not be treated as the entire current form. A postal-address update does not by itself add a State/UT or purpose to the scope of registration under2026 Rule17B.
The26 May2025 amendment expressly adds the relevant-authority approval and prior governing-body-resolution attachments. A changed website address or internally signed memo alone does not replace them.
