When and how this form is used
The2020 Rule12 source misprinted within six months FROM expiry. G.S.R.17(E),11 January2021 officially corrects FROM to BEFORE. For assumed31 December2026 expiry,20 October is within the pre-expiry six-month period; it is not an application six months after lapse.
Under2026 Rule14A,reasonable activity for cancellation/renewal counts FC utilised in the last two financial years and must be at least₹10 lakh. Receipts₹55 lakh in those two years are not the test: actual assumed FC use is₹44 lakh. Keep both tables distinct.
The2026 change requires last-three-financial-year detailed activity reports with the renewal application and adds Government/CAG particulars. The completed model supplies the three receipt rows and separately explains the report package.
No application acknowledgement or ordinary processing period guarantees renewal. Actual grant/refusal,validity and any general-extension order must be checked for the specific certificate; this model assumes an ordinary unextended five-year expiry.
Bank particulars must reflect every actual account. The separate FC-6C/6D change examples do not mean a bank change has occurred in this independent renewal scenario.
