Fictional future hospitality application. No real public official, service credential, invitation, travel sanction or MHA approval is represented.
Fictional, unsigned completed illustration. It demonstrates this form’s fields; it is not a filed return, medical opinion, official order or proof of an actual transaction.
When and how this form is used
Section6 covers the listed public-office categories. Ordinary private travel and a covered officer accepting foreign-paid travel/accommodation require different analyses; the model selects the Government-servant branch.
Rule7’s ordinary advance application is at least two weeks before the proposed journey.20 October→12 December gives53 elapsed days. An application, employer recommendation or host invitation is not itself MHA permission to accept the hospitality.
The22 June2026 amendment adds FC-2 field8A for office/service details. The completed model includes its Government-service branch and marks the other status branches inapplicable. An old unfilled PDF lacking8A is incomplete for this identified current model.
The emergency medical-aid exception and subsequent one-month intimation in the prescribed form note are distinct. The proposed seminar is planned travel, so that exception is not used.
